The “dual plane” placement of the breast implants

When considering breast augmentation with implants, there are many details that need to be discussed with the plastic surgeon during the pre-operative consultation. Among them and probably the most important are the ones about choosing the right type of implant for your body shape.

Deciding upon the shape and volume of the implant is the result of pre-operative measurements. The patient needs to understand that the ultimate goal of the procedure is harmonizing the breasts with the dimensions of the thorax and the body in general. When it comes to breast implants with a round shape, we will also discuss the projection of the implant. The projection determines how many centimeters will make the bust of the patient more prominent. When it comes to the anatomical shape implants, we will take into consideration the width and length of the implant aside from the projection. Last but not least, a decision will need to be made about the volume of the implant. When deciding this, we will take into consideration the weight and height of the patient but also the size of the mammary gland.

Another important decision is the placement of the implants. The implants can be placed in front or behind the pectoral muscles which cover the ribs.

The subglandular placement is an implant behind the mammary gland. We can place the implant in front of the pectoral muscle when the patient has consistent fat tissue and a mammary gland that can easily cover the implant. If the implants are not properly covered by the tissues of the breasts, the submuscular position is required.

When the implants are inserted under the pectoral muscle and completely covered by the muscle, we call this the submuscular position. This type of placement is often used for patients who need small volume implants and have very few mammary gland volumes.

A new, modern technique for breast implants placement is called the dual plane. When this placement is used, the implant is positioned under the pectoral muscle after the relaxation of the muscular attachments to the ribs. The mammary gland is partially detached from the pectoral muscle, so the breast implants are covered by the muscle, but the mammary gland is also arranging itself in a correct manner on the new muscular plan distended by the implant. This is probably the most frequently used breast implant technique placement nowadays as it provides optimal coverage for the implants while also reducing the risks and complications associated with the other types of placement. When the dual plane implant placement is used, the implant is half covered by the mammary gland and half by the pectoral muscle. Most plastic surgeons prefer this superior technique, so make sure to ask about this if the plastic surgeon only presents you the other two types of implant placements.
